We invite UIC students, staff, and faculty to submit descriptions of activities that address the social determinants of health, access to care, primary or secondary disease prevention in Chicagoland communities. . We may contact you to learn more about your program and how it can contribute to UI-CAN reporting. We plan to make shared information publicly available through reports and a self-service directory.
