Our interdisciplinary care team is made up of an expert group of clinical managers; Licensed and Licensed Practical Nurses; medical social workers; certified caregivers; home health aides; and occupational therapists, physiotherapists and speech language pathologists in your own community. Additionally, our agencies work with a medical director who helps evaluate the care we provide and make recommendations for improvement. Our team works closely with your physician(s) to create a plan of care that meets your clinical and recovery requirements and respects your wants and needs. In addition, our team reviews and revises your care plan on an ongoing basis so that it always addresses your concerns. You will always have a copy of your current care plan while you are with us.
Our care team is rigorously trained and selectively selected. All of our employees have passed background checks and interviews, participate in quality reviews and ongoing training, and collectively bring decades of experience in providing home health care. We are confident that our team not only offers top-notch clinical care and expertise, but also a strong character and a reputation you can trust.
