Purpose of the position:
Provides integrated, on-site support to patients and physicians by offering a hands-on approach to total quality patient care through the delivery of a full continuum of medication administration, from providing pharmacology information to monitoring treatment patient medications and the provision of adherence support.
Job duties:
The Clinical Pharmacist will help advance quality drug therapy care by actively ensuring the provision of safe, effective, and cost-effective drug therapy for patients with a variety of disease states. The pharmacist provides telehealth-based individual disease management with patients. The pharmacist will use integrated and routine communication with all healthcare professionals and patients regarding medical care and quality of life, with an emphasis on monitoring and documenting specific drug therapy outcomes. The clinical pharmacist will consult with the patient to identify, resolve and prevent medication-related problems. The clinical pharmacist evaluates each of the patient’s medications for indication, efficacy, safety and compliance issues.
- Management of drug therapy, including reviewing and monitoring drug regimens, advising clinicians on evidence-based therapy as indicated, documenting interventions;
- Ability to review patient clinical record information and determine appropriateness of requested treatment
- Develop clinical criteria and medical policies for drug review;
- Ensure that all clinical criteria are consistent with Food and Drug Administration (FDA) guidelines, clinical guidelines, and appropriate evidence-based literature.
- Manage, organize and update relevant data using database applications;
- Apply observations and recommendations to operational issues, increased productivity, quality and customer service standards;
- Identify trends, solve problems; recommend improvements; implements the change;
- Actively participate in process improvement initiatives;
- Protects the value of the organization by keeping information confidential;
- Updates professional knowledge by participating in training opportunities; read professional publications; maintaining personal networks; participate in professional organizations;
- Desiring to do whatever is necessary for the betterment of the Society;
