The Clinical Pharmacist_Outpatient provides evidence based pharmaceutical care services to include:
• Pharmacotherapy consults
• Disease management support
• Medication reconciliation
• Assessment of polypharmacy issues
• Recommendations to support the patient’s longitudinal plan of care
In certain departments, this position will also proactively conduct analysis of drug utilization data to identify trends, develop strategies and make recommendations for implementing comprehensive programs to improve patient outcomes. Provides committee support as needed for formulary management across the continuum of care.
In certain departments this position will also compound/prepare, label, and dispense medications.

Education: |
Must have working-level knowledge of the English language, including reading, writing and speaking English. Bachelor of Pharmacy or preferably Doctor of Pharmacy Degree. Appropriate education to obtain and maintain Registered Pharmacist licensure in the State of Nevada |

Experience: |
Three years as a practicing pharmacist is preferred. Knowledge and experience in a medication therapy management program is recommended. |

License(s): |
Current and unrestricted state of Nevada Pharmacy License required. |
|
Certification(s): |
Healthcare BLS within the first 90 days of employment is required for all locations. Preferred: Board Certified Ambulatory Care Pharmacist (BCACP) or Board Certified Pharmacotherapy Specialist (BCPS) |
|
Computer / Typing: |
Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc. |
