• Find and set up a primary care provider (PCP).
• Get rides for doctor visits and make appointments.
• Get follow-up care from a provider.
• Understand your discharge plan from a provider.
• Learn about your condition and ways to treat it.
• Find materials so you can learn more about your health and wellness.
