Access the services of your healthcare network

Know your SFHP care network. How your doctors, nurses, physician assistants, counselors and health educators work together for you.

Your primary care provider (PCP)

Your first point of contact for non-urgent, routine and urgent care needs. Call your PCP office anytime, day or night, for medical advice. You should get an appointment for emergency PCP care within 48 hours of your request. Your PCP phone number is shown on your membership ID card.

Pharmacy

When you need medication or vaccines for adults, your PCP will prescribe them for you. Your PCP will send your prescription to the network pharmacy of your choice. Show your membership ID card to pharmacy staff to collect your medication.

Specialists

Your PCP will refer you to a specialist if you need expert advice or treatment for a specific problem or to get a second opinion on your health.

Your hospital / emergency room

When you choose a PCP, you also choose the hospital and emergency room where you can receive care.

Mental health and substance abuse crisis

Get a referral to a mental health care provider or addiction counselor by calling the 24/7 CBHS Access helpline at 1 (415) 255-3737.

Teladoc® Telephone or video visits to the doctor 24/7

If your PCP is not available when you need it and you want treatment for simple medical problems, go to teladoc.com/sfhp, call 1 (800) 835-2362, or download the Teladoc smartphone app.
