SINGAPORE – On Monday (July 5), the Ministry of Health said it was investigating an incident in which a 16-year-old boy had a heart attack after lifting weights six days after receiving his first dose of Pfizer-BioNTech. COVID-19 vaccine.

The ministry said Monday (July 5) that Kho Tek Puat Hospital (KTPH) was informed of the incident on Saturday, as he was treated in the emergency department after his house collapsed the same morning.

The boy was later transferred to the National University Hospital (NUH) in the evening, where he remains in critical condition in the intensive care unit.

Before his collapse on Saturday, the Ministry of Health said he was lifting weights at the gym, adding that it “is aware that he is training with very heavy weights that exceed his body weight”.

The boy received his first dose of the Pfizer-BioNTech/Comirnaty vaccine on June 27.

It added that it was assessed by trained health care personnel to be suitable for vaccination against COVID-19 and was well followed on-site observation after vaccination which lasted about 30 minutes.

The Ministry of Health indicated that he was in good health for the next five days after the vaccination.

“We are in contact with the medical team at NUH who provides close medical care to the patient. The initial diagnosis of his condition is cardiac arrest outside the hospital. Clinical and laboratory tests are underway to understand the underlying cause,” the ministry said.

The ministry added that it will also work with the hospital’s medical team at NUH to determine if the incident was related to the boy’s COVID-19 vaccination. This will include a comprehensive study of whether acute myocarditis, severe inflammation of the heart muscle that affects heart function, is present as a possible diagnosis.

The Expert Committee on COVID-19 Vaccination will monitor the results of the investigation.

New exercise guidelines

“While most people with vaccine-associated myocarditis observed locally and internationally have mild symptoms and recover uneventfully, it is possible that the condition may be exacerbated by factors or strenuous activities that may affect the heart,” the expert panel said in a separate report. Monday statement.

She added that given emerging data about the small risks of developing myocarditis and pericarditis observed after vaccination with COVID-19 mRNA vaccines – such as those produced by Pfizer-BioNTech and Moderna – all individuals who have received any dose of mRNA vaccines should avoid any dose. . Doing sports or strenuous physical activity for a week after vaccination.

This is especially true for teens and younger men under the age of 30.

“During this time, vaccinated people should seek medical attention immediately if they develop chest pain, shortness of breath or abnormal heartbeats. All clinicians should also be vigilant about such clinical presentations after vaccination,” the committee said.

She also said that anyone with myocarditis after receiving the COVID-19 mRNA vaccine should not receive more of these doses.

The panel of experts, citing the Health Sciences Authority (HSA)’s third COVID-19 vaccine safety update released Monday, said there have been 12 reports of myocarditis and pericarditis in individuals after they were vaccinated with COVID-19 mRNA vaccines as of June 30. .

Five cases occurred in adults aged 30 years and over.

The panel noted that the remaining seven included males under the age of 30, “higher than would be expected for this age group, based on background infection rates.”

“While most of the previously reported cases occurred after two doses of vaccination, HSA has also started receiving reports of some cases that occurred after the first dose,” she added.

The expert panel said that after extensive deliberation, it continues to recommend vaccination with COVID-19 mRNA vaccines for all eligible individuals, including adolescents and younger men, “as the preventive benefits from COVID-19 mRNA vaccines continue to outweigh the risks.”

“The use of safe and effective COVID-19 vaccines in Singapore is a top concern of the expert panel, and the panel of experts will continue to monitor domestic and international data to ensure that our vaccination recommendations are updated based on the latest scientific evidence available,” he added.

As of Saturday, a total of 5.71 million doses of COVID-19 vaccines have been administered under Singapore’s national vaccination programme.

About 3.55 million individuals received at least one dose, of whom 2.16 million were fully vaccinated.
